The Ohio District 5 Area Agency on Aging, Inc. is proud to announce a COVID-19 Vaccine and Booster clinic will be held on Thursday, February 24th from 11:00 AM to 12:00 PM, at Hawkins Corner located at 2131 Park Avenue West, Ontario. The Ohio National Guard will administer Moderna First Dose, Moderna Second Dose, or the Moderna Booster to the public.
Anyone 18 and older, and from any county, is welcome to participate in this vaccine clinic. Those interested in participating must make an appointment by calling 567-247-6503. Time slots for the vaccine will range from 11:00 AM to 12:00 PM.
Richland County Transit (RCT) is offering Free Vaccine Shuttle service, thanks to a grant from ODOT. These are door-to-door rides, picking people up at their home and take them to their vaccine appointment. After the vaccine is completed, RCT will pick them back up and take them home. This shuttle service is free for anyone in Richland County. For the free shuttle call 419-522-4504 to make an appointment.
What to bring: Many forms of ID are accepted to verify your name, identity, and age. You do not need to show proof of citizenship or residency status. If you have lost your vaccination card, we can assist you in obtaining a replacement.
Cost: No cost, insurance not required for vaccine or booster.
The Area Agency on Aging realizes it is critical to ensure equity and access to Ohioans who might not otherwise have a chance to get vaccinated, whether their barrier is transportation, location, mobility, or access to care and education about the vaccine.
